Learner Permits

If you are aged 16 to 20 years old, this what you need to bring with you to take your Learner Permit Test:

  • Parent/Legal Guardian
  • Original Birth Certificate (issued by NYS Department of Health, not a hospital certificate) or Passport
  • Original Social Security Card
  • MV-45 form, found here*: *DISCLAIMER* The parent or legal guardian MUST sign the form IN FRONT of a Motor Vehicle Clerk. Pre-signed forms will not be accepted!
  • MV-44 form, found here*: *DISCLAIMER* Applicants under the age of 18 MUST have their parent or legal guardian sign the Parental Consent section.
